After experiencing difficulty navigating our own healthcare journey we created this resource to help others bridge the gap between the hospital and home by translating complex medical information into plain language for patients, caregivers, and families.

Our background includes critical care, emergency medicine, pre and post-surgical care, patient education, utilization management, and care transitions.
